About Us
The Health Center, located in Plainfield, VT, is a primary care practice. As a Federally Qualified Health Center (FQHC), our primary service area includes the towns of Cabot, Calais, East Montpelier, Marshfield, Plainfield, and Woodbury, and we also serve patients from many other surrounding communities. We are a non-profit, community-based health center dedicated to providing quality health care for individuals and families regardless of ability to pay.
The Health Center serves patients of all ages, including families and children, with a full range of primary health care services including medical, dental, mental health, nutrition, care coordination, physical therapy, and a mail-order pharmacy.
