What is an FQHC?

A Federally Qualified Health Center is a primary care practice that provides comprehensive services, including primary medical care, oral health and dental care, behavioral health and counseling services, and patient care coordination.  FQHCs play an important role in health care delivering care that is cost effective, accessible, and affordable. As an FQHC, we offer financial assistance to people who qualify based on household size and household income.  If you have questions about our financial assistance program, please call our billing department at 322-0705. 

As an FQHC, The Health Center receives a federal grant as a part of their annual budget which enables us to offer the sliding scale of fees and community outreach services.  This funding is contingent on demonstrating annually that we meet quality standards.

Out of 101 primary care practices, The Health Center has one of the lowest total cost per patient in the state. Along with cost efficient care, The Health Center has achieved outstanding quality of care results as demonstrated by the Vermont Blueprint for Health Report.

Compared to state-wide measures, The Health Center has:

  • higher levels of diabetes HbA1c testing.
  • one of the lowest rates of uncontrolled diabetes in the state.
  • higher levels of breast cancer screening.
  • higher levels of cervical cancer screening.
  • one of the best rates of controlled high blood pressure.
  • lower levels of outpatient emergency room visits.
  • lower levels of hospital readmissions.
  • lower levels of advanced imaging such as MRI.
