The National Committee for Quality Assurance has designated The Health Center as a Patient-Centered Medical Home.

A Patient Centered Medical Home (PCMH):

• Provides health care that is a partnership between patients and their health care providers.
• Uses established evidence-based protocols of care.
• Models and measures health outcomes.
• Provides a care team to help patients connect with services which cannot be met by The Health Center, at agencies such as:

–  Central Vermont Home Health & Hospice
–  Central Vermont Council on Aging
–  Washington County Mental Health
–  And many others

• Encourages collaboration and teamwork.
• Engages in continuous quality improvement and assurance, demonstrated by periodic analyses and required reports.
