Medical Services
We offer a wide range of primary care services for patients of all ages, including preventative care, management of chronic conditions, and acute care. We also have a neurologist, dieticians, and physical therapy services available on site.
High Quality of Care
Compared to state-wide measures, The Health Center has:
- Higher levels of diabetes HbA1c testing
- One of the lowest rate of uncontrolled diabetes in the state
- Higher levels of breast cancer screening
- Higher levels of cervical cancer screening (among the top two in the state)
- One of the best rate of controlled high blood pressure
- Lower levels of outpatient emergency room visits (second lowest in the state)
- Lower levels of hospital readmissions
- Lower levels of advanced imaging such as MRI
The Health Center is a Patient Centered Medical Home
The National Committee for Quality Assurance has designated The Health Center as a Patient Centered Medical Home at the highest level (Level 3).
A Patient Centered Medical Home (PCMH):
- Provides health care that is a partnership between patients and their health care providers.
- Uses established protocols of care.
- Models and measures health outcomes.
- Provides a care team to help patients connect with services which cannot be met by The Health Center, at agencies such as:
– Central Vermont Home Health & Hospice
– Central Vermont Council on Aging
– Washington County Mental Health
– And many others
- Encourages collaboration and teamwork.
- Engages in continuous quality improvement and assurance, demonstrated by periodic analysis and required reports.
